Sometimes I think your problem in 3-min is that you just don't focus. For example in the last game of this video your opponent had a long think and played Re1, which set up the easy threat of Nxg4 and Rxe7. However you spent all of this time discussing how bad Yaacov's opening was in the prior game and missed the idea.
